NO PURCHASE NECESSARY.
1. Contestant must be 18 years of age to enter.
2. Contestants must reside within the KTVL News10 viewing area.
3. Entry forms must be filled out completely and be received between October 29 through November 24, 2009.
4. One (1) winner(s) will be chosen on November 25, 2009. The winners will be contacted by email. The winner will be selected by voting on KTVL.com. The story that receives the largest number of votes will be determined as the winner.
5. The winner will receive a Visa debit card from Rogue Federal Credit Union including $1,000 in funds.
6. Acceptance of this prize constitutes permission to use the winner's name and likeness for promotion, advertising and publicity purposes at no additional compensation. No transfer or substitution of prizes is permitted.
7. Employees of KTVL News10 and SOUTHERN OREGON CW are not eligible to win.
8. KTVL News10 and SOUTHERN OREGON CW shall not be liable for lost or stolen entries, personal or property injury, losses or failure of providers in connection with awarding of this prize.
9. Contest is void where prohibited.
10. All decisions are final.
11. All tax, insurance, gratuities, and other liabilities are the responsibility of the winner, unless otherwise stated.
12. One entry per family.
13. Contestant must reside in KTVL’s viewing area.
14. All photos submitted become the property of KTVL.
15. After 30 days, any unclaimed prizes will revert to the station and may be awarded to another contestant or retained as we deem appropriate.
16. The Contest and all of the related pages, contents and code are copyright, UPICKEM, Inc., and/or KTVL. Copying or unauthorized use of any copyrighted materials, trademarks or any other intellectual property without the express written consent of its owner is strictly prohibited.
17. Each winner, by acceptance of the prize, agrees to release all Sponsors, and their parent and subsidiary companies, their officers, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the receipt, ownership, or use of the prize.
18. UPICKEM and KTVL are not responsible for Internet crashes or slowdowns caused by network congestion, viruses, sabotage, satellite failures, phone line failures, electrical outages, natural disasters or acts of man or God.
19. All results posted are unofficial until winners respond to e-mail notification.
20. Each winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received.
21. This contest is not intended for gambling. If it is determined that a contestant is using the contest for gambling purposes they will be disqualified and reported to authorities.
22. We reserve the right to change contest rules at any time.
